How to access your patient's health records

The Health Practitioner Portal is also known as HPP or The Viewer.

The HPP provides information for GPs about each patient who receives treatment or care at a Queensland Health facility.

It includes:

• pathology results
• radiology results
• medications
• allergies and alerts
• discharge summaries
• outpatient letters.

You can use the HHP if you have:

• a QGov account
• active profession practice details.

If you don't have access to the HHP, you'll need to contact the health centre or hospital directly.
